Session Objective:
Introduce and discuss the implementation of a portfolio, the "Fitfolio", to help students set goals for their fitness tests while preparing them for the 5th Grade Classroom Based Assessments.
This presentation provides specific examples of a student-centered portfolio designed to meet the Health and Fitness EALRs and 5th Grade Classroom Based Assessments (CBA): Concepts of Health and Fitness, Get Fit Summer, and Stomp Out Second-hand Smoke. The Fitfolio can also be easily adapted for the CBA, Mrs. Trimble's Muffins.

The 4th Grade "Fitfolio" features a writing section, "My Fitness Journal" where students develop a goal and a plan to improve on each of five fitness tests (1/2-Mile Run/Jog, Shuttle-run, Curl-ups, V-Sit, and Push-ups) that we do at Blaine Elementary (click here for Units). Rubrics are based on the 4th Grade Writing Content, Organization and Style, or COS. In addition, there is a featured section that invites students to participate in a poster contest on Kick Butts Day, "Stomp Out Second-hand Smoke.
The 5th Grade Fitfolio(TM) also features a writing section for "My Fitness Journal" for each of the five fitness tests conducted throughout the school year. Within the "My Fitness Journal" along with realistic goal-setting and planning students also have to identify barriers that could affect their plan, and strategies to over come their barriers. Four vocabulary assignments are also given to help students become more familiar with the various health and fitness words that they will encounter within the Fitfolio(TM), class discussions, and the CBAs. For both the 4th and 5th grade Fitfolios(TM) including the section "MyFitness Journal" and the 4th grade "Stomp Out Second-hand Smoke" Contest I included identical verbiage and prompts as students will find in their CBA's.
All Fitfolio(TM) is now a trademarked, and the written material (.pdfs, Fitfolio(TM) folders, etc.) for both 4th and 5th grade is copyright protected.

Some tips to consider when implementing your Fitfolio(TM) for the first time:
- Try just one class, and grade level such as 4th Grade Fitfolio(TM) first. That way, students the following year will be prepared for the 5th Grade Fitfolio(TM) which features more sophisticated goal-setting techniques.
- Assign Fitfolio(TM) homework right before the weekend to give students an opportunity to work on the assignments over the weekend.
- Send information to classroom teachers the day of the assignment regarding the due dates.
- Provide students with an "Assignment Due" sheet to help them "remember" what is to be done.
- Provide the teachers with copies of the LEAP OSPI 5th Grade Health and Fitness Vocabulary (click here) as a reference source for your students.
- Set up a Health and Fitness resource section in your library and/or website that features the LEAP 5th grade vocabulary list and resources for Stomp Out Second-hand Smoke.
- Decide ahead of time your consequence, if any, for students who will not be turning their assignment in on time. For this purpose student either come to PE class with their homework OR their planner. For students not returning homework on the assigned due date I attach to their planner a "Dear Parent" note indicating my expectation for a final return date (1 point is lost out of 4 for late assignments)
- It helps to ensure you've made it clear in various ways that ALL students should have Fitfolios(TM) returned by a Friday so that you can grade ALL Fitfolios(TM)over the weekend (which takes me roughly 3 hours to correct and record all 320 Fitfolios(TM)).
- Know ahead of time your students of special needs (ELL, 504's, SPED) and prepare to accommodate by adding a page in the students Fitfolio(TM) asking them to work on this homework with their specialist).
- Use of an overhead with transparencies of the intended assignment(s) during introduction of assignments - this helps tremendously.
Additionally, I use a palm-pilot within the PE setting to quickly conduct assessments for data input into student report cards was presented. I am able to quickly do assessments, and keep track of rubric scores by using a Tungsten T5 (or any simple Palm Pilot, or other PDA will do) and the software, "Documents To Go".
If you are not interested in developing your own report card then the software, "Learner Profile 3.0 To Go" has been well tested and approved by our librarian and music teacher here at BES for their report cards.
An example of a student PE report card, Grade 5 in .pdf format, and Grade 4 in .pdf format was used on the Apple works database. The featured report card highlights the EALRs / GLE's, and the CBA for Health and Fitness, Grade 5.
